Patients often ask me what makes a smile beautiful. Because this question comes up so often, I decided to write a blog about the esthetic components of a well-detailed smile. Today’s topic: Smile Arc. Smile Arc describes how the biting edges of the upper teeth ‘flow’ along the curvature of the lower lip line. When this arc is present, smiles are called ‘consonant’ and judged more esthetic.
Justine had braces with us for 11 months. Her final photos (figs. 1,2 below) show beautifully straight teeth framed within her smiling lips: a consonant smile arc.

I placed Justine’s braces so that her top teeth would flow with her lower lip curve. You can see how the lateral tooth (fig. 3, green arrow) will be ‘extruded’ downward by the flexible wire until the smile arc (figs. 2,4) is complete.

Traditional orthodontic treatment has been great at aligning the teeth and achieving a stable and functional bite. The sad truth, however, is that this doesn’t necessarily always result in a great smile. Modern orthodontics and more progressive orthodontists such as Dr. Benson also evaluate how the teeth are framed by the lips and position the teeth such that they provide optimal lip support as well as maximizing tooth display when smiling.
While both these patients have straight teeth and a good bite, you can see how much more aesthetic the smile for the patient on the left looks because of the increased tooth display, wide smile, and curvature of the smile arc. This is generally more desirable than the flattened, narrow smile on the right.
While Dr. Benson’s goal is to always align your teeth and give you a stable, functional bite, her primary goal is to give you a beautiful smile such as the one pictured above, utilizing a treatment philosophy called Smile Arc Protection (SAP). How do we achieve Smile Arc Protection?
You may notice that traditional orthodontic treatment involves placing the bracket in the center of the tooth. This is great to align the teeth, but it often has a negative side effect of flattening the smile arc. With the innovative Smile Arc Protection technique, orthodontic brackets are placed on the teeth closer to the gumline in order to create more appealing, aesthetic results by extruding the anterior teeth. By positioning the brackets in this way, patients can receive a broader smile with increased teeth visibility rather than a flattened traditional smile. At The Art of Braces in Las Vegas, NV, Dr. Saoly Benson utilizes Smile Arc Protection (SAP) when treatment planning for all of her patients receiving traditional braces, including clear, ceramic braces as well as removable clear aligners such as Invisalign.
The treatment process for SAP is similar to the placement of brackets for traditional metal braces. However, the brackets are placed closer to the gums for optimal results. Prior to the placement, your teeth with be cleaned and polished. We will go over these instructions with you at your appointment as well as discuss which foods to avoid as well as foods that are safe to eat. Additionally, you will need to attend follow-up appointments with Dr. Benson so that she can evaluate your treatment progress and make adjustments as necessary.

Many people know Invisalign for its ability to straighten teeth and create a more beautiful smile. However, Invisalign treatment can also be used to create a wider, more radiant smile.
Smile widening is the process of changing the shape of the mouth, or moving the teeth, to create a wider smile. Sometimes, it can be as simple as shifting overcrowded teeth to a more natural posting, allowing them the correct amount of spacing. Other times, however, it can be a bit more complicated.
What is a narrow smile?
A narrow smile is noticeable when only a few front teeth are visible when smiling. If six or fewer teeth are visible when smiling, it is generally considered to be a narrow smile. While narrow smiles don’t often cause any sort of medical problems, many people want to have their smile corrected to make it wider and more appealing. There are times, however, when a narrow smile can create larger problems.
A narrow smile is often caused by a misshapen maxillary arch (the upper part of the mouth). The maxillary arch is supposed to be curved, giving it the traditional U shape that allows teeth to grow into a curved pattern. If the maxillary arch tapers and starts to take on more of a V shape, the opening of the mouth narrows. Teeth can then start to tilt inward, and there ends up being extra space between the teeth and the cheeks. This can create a narrower jawline, and the cheeks can even start to appeal sunken-in.
In severe cases, a narrow smile can accompany other dental problems like overbite, underbite, or crossbite. If the jaw alignment is off so that the mouth cannot close correctly, the patient could develop a speech impediment or could even suffer from difficulty breathing.
Can a narrow smile be fixed?
Fortunately, a narrow smile can be fixed. Most people with a narrow smile do not experience medical problems and do not need medical intervention. However, it is common for people with a narrow smile to want it fixed for cosmetic reasons. A wider smile is often seen as more appealing and can make life easier.
Depending on the severity of the narrow smile, the patient might need surgery or might be able to use a less invasive procedure like traditional metal braces or Invisalign. For children under the age of 13, many orthodontists will use a palate expander to widen the maxillary arch before it is fully formed. After the age of 13 though, the arch is no longer malleable. For adults with an overly narrow maxillary arch, surgery is needed to expand the palate to the correct width.
If the maxillary arch is wide enough that it does not need to be expanded, or if palate-expanding surgery is complete, Invisalign can be used to shift the teeth into the correct position and widen the smile. Invisalign does not actually change the shape of the palate or the jawbone. It merely shifts the teeth once the other corrective action has already taken place. Once the teeth have been shifted into a wider position, the smile will appear wider. Even minute shifts can have a huge impact on the smile.
If you are considering Invisalign treatment for smile widening, schedule a consultation with your orthodontist. For mild adjustments, Invisalign can be a discreet alternative to traditional metal braces. However, metal braces can be a better fit for more severe spacing problems. Your orthodontists will be able to help you find the right solution for your particular situation.
How does Invisalign widen a smile?
Many people prefer Invisalign to traditional metal braces. Invisalign is more discreet, less painful, and easily removable. The cost is comparable to metal braces in most cases. However, Invisalign is not for everyone. Invisalign tends to work well for minor spacing problems like overcrowding, small gaps, or slightly crooked teeth. For cases of moderate to severe gaps, overcrowding, or crossbite, metal braces are often a better choice.
If Dr. Aller agrees that Invisalign is right for you, here’s what you can expect:
Invisalign treatment length can last as little as 6, or as long as 18 months, but results are seen along the way. You will receive clear plastic aligners designed to fit snugly over your teeth. Though you will need to remove the aligners while eating and drinking, you will wear them for the majority of the day. The aligners gently move the teeth into position over time to create the desired result — a wider smile and a better bite.
While your smile is widening, you can expect your upper teeth to shift slightly outward. If your teeth were previously crowded, they will begin to space out to sit side-by-side. Once your teeth are properly spaced and shifted, your smile will appear wider.

Can Higher Bracket Placement Create Better Smile Arc?

“The placement of the brackets depends on the smile arc of the patient,” says Dr. Gire. “The ideal smile arc is when the edges of the top teeth follow the curve of the lower lip. To achieve this, we place the brackets slightly higher and with a gradual, natural step in heights. This may be something that you may not be used to seeing, but the higher bracket placement guarantees the integrity of the smile arc.”
The concept of the smile arc is not a new one, however, not all orthodontists are as dedicated to ensuring the ideal smile arc is provided once treatment is complete. Some studies have shown that orthodontically treated patients can end up with flatter smile arcs than non-treated patients. This can be for a few reasons, including:
Attempting to correct an overbite or gummy smile without also considering the relationship between the incisor and lip while at rest.
The use of extra-oral forces, such as elastics or those that undergo orthognathic surgery, cant the maxillary occlusal plane upward, which will then move the upper incisal edges away from the lower lip.
Incorrect bracket positioning, defined as positioning that does not take into account the current smile arc of the individual patient.
“Achieving the ideal smile arc should be individualized for each patient,” says Dr. Gire. “We cannot use the exact same bracket positioning with every patient, just as we cannot use the same exact treatment plan with every patient. Each treatment plan MUST be customized for each patient because each smile is as unique as a fingerprint. My primary goal is to not only get the teeth straight, but I want that natural smile arc to be preserved or created, and I want the patient’s face to frame that smile. There’s a lot of moving parts and there’s no way this can be a cookie-cutter type of treatment.”

This case was relatively straight forward. The patient decided not to have their lower teeth treated, but the change is still dramatic. Treatment improved the alignment and shape of the arch. Case #2 – Alignment and Arch Expansion – Butterfly Incisor Before & After
This was also a relatively simple case for clear aligners. Treatment improved the alignment of butterfly incisors and the shape of the arch. This type of case requires adequate retention to ensure the rotated teeth do not relapse.

Both upper and lower teeth have crowding. Invisalign successfully achieved expansion of the narrow arches and better teeth alignment for an improved smile. Treatment was completed in approximately 12 months. Invisalign treatment of an overbite (especially when severe) is less predictable than with braces.

Case #6 – Severe Overcrowding, Arch Expansion and Teeth Alignment Before & After

This case is an ideal for Invisalign treatment because the bite was already good. Due to severe overcrowding, a lower incisor tooth was extracted to provide the extra space the bottom teeth needed. The upper lateral incisors are small so the mouth still balanced out by removing the lower incisor. Expansion of the arch and teeth alignment was achieved with this Invisalign treatment. This treatment took around 24 months to complete.
Teeth that are rotated before treatment are particularly prone to moving back to their old position after treatment has finished. The patient wears a fixed retainer behind the upper arch to ensure there is no movement of the teeth as they age.

How Long Does the Treatment Process Take With Invisalign?
Invisalign treatment time can vary from as little as six months to two years. While every treatment is slightly different, the long-term success of patients is often determined by what happens after treatment.
It is critical for patients to understand that it takes years for bone and gum to stabilise after orthodontic treatment. While their teeth may look great after their Invisalign treatment has ended, most patients will need a long-term strategy to keep them looking that way.
Teeth have a natural tendency to return to their original position or crowd with time. Fixed wire retainers behind the bottom, top or both teeth holds them in the correct alignment. Retainers worn every night will also keep teeth in position, so the patient enjoys a successful long-term outcome.

Invisalign vs Braces Effectiveness
